Appointments

You can make appointments on line 24 hours a day, 7 days a week. Simply ask the staff on reception for a simple form to sign and complete and let us have your identification with a photo on it and we will give you a username and password that will enable you to book appointments on line. The system is very easy to use and there are appointments for each Dr each day, so you should have no problem seeing a Dr of your choice at your preferred time.

You will need a user name and password for each family member though, so please remember this when you fill in your forms and bring in your ID. All patients over the age of 18 will need their own ID and form.

You may not always need to see a clinician face to face, for speed and convenience we offer over 70 telephone slots per week - Please ask at reception or online for a telephone appointment if required.

Registration

If you have not yet registered at our practice you will need to visit the practice and complete a registration form.
Once you have completed the form, please return to the surgery before booking an appointment.

When coming into the practice to register, ensure you have the following information / documentation:

  • Name
  • Address
  • Date of Birth
  • Name and Address of previous doctor
  • NHS Number / Medical Card
  • 2 forms of identification

If you require assistance with your registration, we can arrange a registration medical appointment for you, also if you prefer a particular gender of doctor (male/female), please make us aware and we will endeavour to accommodate. If you do not have a fixed home address, please inform reception staff and we will still be able to assist even if you are homeless, a traveller or newly entering the United Kingdom.

Home Visits

If possible, please try to telephone before 10am. A doctor or nurse may phone you back as it may be that your problem can be dealt with over the telephone or that it may be more appropriate to send a nurse or indeed arrange a hospital attendance.

Household visits are only available for patients who are housebound because of illness or disability
